Actually, this has happened three times before. Three players sent off in the World Cup, it has happened now a total of four times. Never have as much as four players been sent off, though - that would have been a new record.
For the record, the first card was correct and applause to the ref for that. The second was harsh. The third was correct. I think the ref lost a bit of control; the match varied in pace so much that he seemed to lose his concentration during various phases. A penalty should definitely have been awarded to Italy for that holding inside the box.

When evaluating a tackle, the ref needs to take into consideration the following things:
- The tackler's speed/intensity.
- The tackler's position (on ball/on opponent).
- The tackler's chance to hit the ball.
- The tackler's intentions (is he trying to reach the ball?).
- Is the player's safety at risk?
In this case, the tackle was made before the ball reached the Italian player. The American therefore had no chance of knowing whether he had a chance to reach the ball. He tackled anyway, putting the player's safety at risk. This would suggest a red card, but the fact is that the tackler did hit the ball (as well as the player). In my opinion, points 2-4 become irrelevant at that point. The only thing that matters is the intensity of the tackle and the Italian's safety. Seeing the situation in replay doesn't help; you need to see it "live" and not in slow motion to be able to judge the intensity of the tackle. Doing that, you'll get an impression of what the tackle constitutes: a ruthless act or a dangerous one. There lies the difference between yellow and red.
Here, the rulebook is a safe haven for referees, because it is left to them to decide:
"Ruthless act: probability of yellow card increases. Excessive force: probability of red card increases".
So the referee is on safe ground either way, it all depends how he judges that tackle. The problem is that this was not the only tackle like this. Yet the ref didn't awards any of the others with a red card...
I think his biggest problem will not be the cards; he will certainly receive some praise for catching that first one. His biggest problem is not seeing the holding of Iaquinta two minutes from time. That should have been a penalty.
